Abstract

In a paper published some ten years ago the view was advanced that serious accident investigation should be undertaken as a research project, with the object of elucidating, not only the circumstances of the accident itself but also any new knowledge exposed. During the subsequent decade, new incentives to more and better investigations have emerged and responses have included new and improved techniques within the general approach advocated. Some of these are discussed.
