Abstract

Jayne is a 28 year old unemployed single mother of two who has attended hospital after calling an ambulance and stating she had taken an overdose. The paramedics found her at home in bed next to two empty packets of paracetamol andshe willingly attended A&E. Her paracetamol levels were elevated and she received treatment with N-acetylcysteine.She has had five medical admissions after intentional overdoses so far this year. She has a history of repeated presentations to A&E, initially with self-cutting in her early twenties followed by numerous insignificant overdoses over the last couple of years. She has previously declined psychiatric assessment on a number of occasions. On the few occasions she has agreed to a mental health assessment in hospital, she has not been offered admission and has declined community psychiatry input, opting instead for support from a local voluntary women´s group. She has been given a diagnosis of emotionally unstable (borderline) personality disorder and has a history of some kind of childhood trauma.
