Abstract

Human fallibility presents a significant threat to the operation of complex technological systems. The distinction between errors of automatic and controlled processing has important implications for system safety and reliability. Robust systems generally require a combination of automatic and controlled errors to occur before a serious system disturbance is possible. An understanding of the cognitive forms of error, and the manner in which multiple errors combine to threaten complex systems enables the formulation of a standard against which the resilience of complex technological systems can be evaluated.
