abstract="Many companies produce lengthy analyses in which accident rates, measured in various ways, are compared with age, service, cause, nature of injury, part of body injured and so on. Few conclusions, if any, are drawn from these data and little or nothing is done differently as a result. Trends within a company can usually be detected by perusing accidents reports. Instead of collecting most of the data we collect at present, we would do better to direct our efforts towards the collection of data on equipment reliability and human reliability.<p />",
