abstract="This article summarizes the case investigation of the death of a 37-year old diesel mechanic and rail transportation employee who died of multiple blunt force injuries after his utility truck collided with the rear of a tractor-trailer on a four-lane interstate highway. The article describes the victim's work with the transportation company, the employer's written safety programs and training, the incident scene, the crash investigation and findings on cause of death. Highlighted is the discovery of a cell phone in the victim's hand and evidence of text messages exchanged prior to the crash. In addition to a detailed description of the crash investigation, the article presents recommendations for safe driving practices and employer safety policy.
