abstract="Inter-hospital transfer (IHT) may lead to poor outcome due to delay of surgery in severe traumatic brain injury (TBI) patients. We assessed whether IHT leads to early death in patients with isolated severe TBI requiring decompressive craniectomy.   Data from TBI patients requiring decompressive craniectomy at two emergency medical centers between 2015 and 2018 were retrospectively analyzed. Among 164 eligible subjects, 78 subjects with GCS score >8 on admission (n = 51) and delayed surgery followed by late deterioration (n = 27) were excluded. Finally, a total of 86 subjects were enrolled in the present study. . The patients were divided into two groups. The patients directly presented to a Level 1 trauma center or university hospital were defined as no IHT group. Patients who were transferred to Level 1 trauma center or university hospital after initial diagnosis were defined as IHT group. The primary outcome measure was 24- h mortality.
