abstract="An analysis was made on 700 cases reported as incidents in the year 2002. They consisted of 343 cases of stumbling and falling (49%), 246 cases of incidents related to injections and internal medicines (35%) and 111 cases of the other incidents (16%). The incident of stumbling and falling was noted mostly in elderly persons. It was liable to happen when they were in toilets. Its incidence was high in the nighttime. Its high incidence was particularly remarkable in our hospital. As regards the incidents caused by injections and internal medicines, there were 19 cases (7.7%) which were treated erroneously. Each patient has been asked to wear a wristband since May, 2003 in this hospital, and it is expected that such error will never be repeated in the future. Besides, it was occasionally forgotten to give drugs to patients at prescribed time, and errors happened now and then on dosage, administration route and administration velocity. In this connection, it is necessary to take preventive steps by means of making thorough re-confirmation, clarifying instructions and making double check. As far as the other incidents were concerned, there were 5 cases of the level 5 (dead cases). Two of them were cases of cancer which committed suicide. Two others were related to treatment and surgical operation. The Risk Management Committee is making continued efforts to establish a regime to provide safety medical care, through making good analysis of incidents and laying out countermeasures.<p /><p>Language: ja</p>",
