%X The CARE Communities Project was developed, implemented, and evaluated through the collaborative and collective efforts of public health practice professionals serving in academic and practitioner roles.  This report describes the roles of academic and practice partners and the collaborativeprocessinvolved in procuring funding, program and evaluation development and implementation, and interpretation of findings.  Facilitators and barriers to this work, as well as benefits to all partners, are also described.
