%X In response to our agency exceeding the national benchmark rates for "Emergent Care for Injury Caused by Fall or Accident at Home," Alliance Visiting Nurse Association & Hospice developed a program to assess, prevent, or decrease the risk of falls in our mostly elderly and/or chronically ill population. Using the Tinetti Assessment score as the starting point, we developed a program of assessment, education, and follow-up that significantly reduced the number of patients requiring care for fall-related injuries.
