%X Joseph Vacher the famous serial killer was judged responsible for his acts and guillotined at the end of the 19th Century. Vacher's brain was examined by eminent scientists who hoped to find evidence to account of the killer's mental alienation. The discovery that one of his sisters died from pre-senile dementia, which initially presented as dysthymic disorder, suggests that Vacher may have been a carrier of a hereditary disease, and lead to this review of his medical file. This article presents evidence for a diagnosis of a schizo-affective disorder and, according to current knowledge, of a lesion/dysfunction of the temporal lobe.<p />
