The present paper attempts to integrate various aspects connected with safety of systems and faults in the equipment and sensors. Except for dust explosions in grain stores and other processes, most other serious accidents have involved emissions from point failure such as valves, seals and pipe connections. Weak links known to have caused past accidents are emphasized and some suggestions made for accident prevention. Sensors have been identified as a weak element in a safety system and various aspects are discussed. Fault detection, especially for equipment, sensors and actuators. Various procedures are reviewed. Early detection of symptoms leading to an accident is important, and therefore integrity of all elements in a detection system has to be maintained.
