Failures often occur because of weaknesses in the understanding of critical tasks, compliance with procedures, leadership, communications and so on. By focusing on improving these areas, research has shown that an organisation can improve its safety performance. However, establishing a successful management approach requires leading indicators of the maturity and effectiveness of management activities. This paper presents a framework for Organisational Integrity that can be applied to assess performance management, identify areas for improvement and reduce the risk of failure occurring. The details of the framework are set out, including an explanation of the 8 critical dimensions of assessment. We describe the method of application and provide examples of the data outputs from the method applied.
