N2  - Changes in the psychiatric services are the hallmark of the last two decades in the UK, USA and Europe. The move towards community-oriented services is the prominent change in service delivery. The numbers of hospital beds are being reduced, and psychiatrists are coming to the realization that they cannot function within a 'purely' medical domain. However, life in the complex community situation has dangers as well as advantages. Suicide, criminality, rehospitalization and homelessness need to be carefully evaluated. At the same time patients' quality of life probably improves. The present critical review addresses the advantages and dangers inherent in the so-called 'deinstitutionalization' movement through the 'least restrictive environment' point of view.<p /> <p>Language: en</p>
