Hospitals must keep records and data which will provide public health officials, industry, and those responsible for sports programs a means of identifying and eliminating causes for injury.    The design and considerations which we conceived important in developing an Emergency Department record responsive to the needs of hospital and community planning of The University of Michigan Medical Center are reviewed here.<p />
