Abstract

In order to prevent and reduce accidents caused by human errors, the root cause of human errors was found by analyzing different types of human errors. The 4M barrier theory was used to find out the failure barrier, and repair methods were proposed. On the one hand, power generation enterprises should play the role of guidance, cohesion, radiation, and assimilation of safety culture. On the other hand, they should guide and standardize safety production activities, make full use of human error prevention tools, and prevent human errors before the event. Through case analysis, tools to reduce human errors were discussed. These tools include star self-inspection, questioning attitude, three-way communication, compliance with procedures, monitoring, independent verification, pre-job briefing, suspension in case of uncertainty, work handover, and other inspections. The results show that if tools to reduce human errors are used, safety education and training are strengthened, and human-machine safety conditions are improved, the chain of accidents can be effectively cut. In addition, the barrier loopholes can be successfully blocked. Furthermore, the above problems can be solved by enhancing safety management and building a multiple prevention system based on the 4M barrier theory, so as to avoid accidents. Looking forward to the future and building an excellent safety culture serve as the core logic of transforming passive safety into active safety. Moreover, a high degree of self-consciousness and self-discipline in safety work can be seen among all staff by improving their safety concept, consciousness, thought, attitude, knowledge, and skills. © 2023 Zhongguo Zhongyi Yanjiuyuan.
