Abstract

A classification system has been developed to provide a logical method for the dissection and reconstruction of malfunction scenarios, and to identify the constituent elements of a scenario in a concise and unambiguous terminology. The classification system is described in some detail, and its application to the analysis of operational malfunction events in nuclear plant auxiliary feedwater systems is illustrated. It is suggested that the classification system approach and terminology be adopted and used to interpret and communicate malfunction scenario information and statistics--and in particular that is be used in lieu of the common-cause failure terminology.
