Abstract

On February 28, 2004, the Bow Mariner, a chemical tanker, caught fire, exploded, and sank off the coast of Virginia. Only six of the 27 crew members onboard survived. The article describes the findings of the marine casualty investigation on events leading to the tragedy and its likely cause, as well as Coast Guard search and rescue and commercial vessel response. A box describes cultural and cohesiveness issues onboard the Bow Mariner which likely contributed to the incident and definitely contributed to the high casualty rate.
