Abstract

This article details the National Institute for Occupational Safety and Health (NIOSH) report of an accident on October 17, 2003 involving a 28-year old male volunteer fire department probationary member. The individual was killed after the pumper/tanker he was operating ran off the road and overturned. The report describes the victim's training and experience, the fire department's equipment and personnel, the weather and road conditions at the time of the accident, and the timeline for the events associated with the crash. It also details the investigation, crash reconstruction and factors contributing to the crash. Recommendations are offered to prevent further incidents of this type.
