Abstract

Presents the National Transportation Safety Board (NTSB) report on a median crossover multivehicle accident that occurred on January 7, 2013 in Houston, Texas. Three vehicles were involved -- a 2004 Toyota Camry, a 2005 Lincoln LS, and a 2000 Toyota Avalon -- in a crash that resulted in one fatality and one serious injury. This report presents a description of the crash, roadway and vehicle information, outcomes of interviews with the Camry driver and her coworkers, and an analysis of contributing fatigue factors. The probable cause of the crash was determined to be the Camry driver's failure to maintain directional control of her vehicle due to work-related fatigue.
