Abstract

This National Institute for Occupational Safety and Health (NIOSH) report describes an accident involving a volunteer fire fighter who died when the tanker he was driving to a grass fire crashed in a semi-rural part of Indiana. The report offers a detailed description of the victim's fire department, training and experience; the equipment and personnel involved; the crash investigation; and contributing factors to the crash. The report offers seven recommendations to prevent similar accidents and outcomes, based on the findings of the investigation.
