Abstract

This N.I.O.S.H. report excerpt presents findings from the investigation of an accident involving a fire department pumper that crashed into a rock outcrop in West Virginia. The crash resulted in the death of two firefighters and the injury of three other firefighters. The report details the victims' training and experience, the crash timeline, and conditions impacting the crash. Additionally, the investigation, crash reconstruction, contributing factors and cause of death are examined. Seven recommendations stemming from the findings of the investigation are presented.
